Diabetes Peer Mentoring for Home Care Workers

Home Care members successfully managing their own diabetes were identified and trained as Peer Mentors by InquisitHealth in self-management principles, coaching skills, and the use of Mentor1to1™, InquisitHealth’s web-based mentoring platform.

These Peer Mentors deliver a 1-on-1, phone + smartphone-based mentoring program to other members with poorly-controlled diabetes.

Because Home Care workers face unique challenges to diabetes management, peer mentoring is uniquely suited to identify and address multiple underlying challenges to better health.

This program has consistently led to a 1+ point reduction in HbA1c, a clinically significant improvement linked to reduced microvascular complications and lower medical costs.
