Our technology platform has helped the National Kidney Foundation offer robust peer support to patients with late-stage kidney disease who are on dialysis or who are awaiting a kidney transplant.

Over 150 peer supporters have been matched with over 750 successful patients. 97% of patients found the program’s support helpful.

And the program has demonstrated a dramatic 14 point improvement in the Patient Activation Measure (PAM) score! Each point increase ties to a 2% reduction in hospitalizations! (Insignia 2018)

Partner Story

ViiV Healthcare, a UK-based HIV pharmaceutical company, partnered with us to develop, deliver and manage all aspects of an unbranded HIV peer support program.

We have helped lead technology development, peer supporter training, community management, as well as partnership development!

Over 30 trained peer supporters have helped over 1200 patients living with HIV dealing with poor medication adherence, challenges with disclosure stigma, and the social- and behavioral- determinants of health.
