Peer-to-Peer Support for Dialysis and Kidney Transplant

PeerStrong™ allows NKF to manage their national network of trained Peer Supporters and match patients and families with these Peer Supporters based on shared experiences with dialysis, transplant, and/or living kidney donation. The program facilitates 1-to-1 support and communication between Peers and Peer Supporters via phone call, chat messages, and participation in a community forum. The NKFPeers™ program has demonstrated a 12-point increase in the Patient Activation Measure (PAM) score, which correlates with a 2% reduction in hospitalizations and a 2% increase in medication adherence.
