Parent Mentoring for Pediatric Asthma

The parents of Healthfirst members with asthma are matched with parent mentors who are certified asthma educators and parents of children with asthma themselves. Through phone calls and smartphone-app based content and communication, mentors identify and help address the unique challenges each parent faces in managing their child’s asthma. In addition, asthma mentors can help address social determinants of health by connecting parents with trusted community based organizations (CBOs), integrated pest management services, pharmacy support, food insecurity programs, and more. The focus is to empower each child and family to better manage their asthma to avoid exacerbations requiring ED visits and hospitalizations.
