National Sickle Cell Peer-to-Peer Mentoring Program

InquisitHealth has helped develop a (a) comprehensive sickle cell disease training program for mentors (who have successfully completed their own transition from pediatric to adult sickle cell care), and (b) a custom technology platform with the tools necessary for effective, 1-to-1 peer mentoring. Trained peer mentors are matched 1-to-1 with patients to offer individualized support, education, and the resources necessary for a successful transition.
