Diabetes Peer-to-Peer Mentoring with Public Hospital System

Peer Mentors are trained by InquisitHealth and matched with patients struggling with diabetes on the basis of shared experience and other demographic factors in order to provide empathetic, culturally sensitive support to achieve chronic disease management goals. Utilizing InquisitHealth’s technology platform, Peer Mentors extend the reach of clinical teams to coach patients in self-management behaviors, help them navigate the healthcare system, and help address their social needs.
