A huge challenge in addressing the social determinants of health is patients not following through. Why?

We keep asking those struggling to lift the biggest burden.

Current strategies are necessary, but just not sufficient:

– Standardized coding of social needs

– EMR-integrated referrals for social services

– Closed-loop referrals

All important, but just not enough. What we are missing/need:

– More proactive outreach

– Less bureaucratic requirements

– More empathy

The more we can reduce burden for the patient, the more successful we will be in helping them address SDoH-related barriers.

TL;DR: We need more ‘social’ support for those struggling with the ‘social’ determinants of health.
